An outbreak of P. falciparum malaria occurred aboard a Norwegian merchant vessel sailing from West African ports to the United States. Twelve cases with one death occurred among the 40 persons aboard the ship. Chemoprophylaxis was carried out only sporadically. Clinical characteristics varied widely and included asymptomatic infection, influenza-like illness, and cerebral manifestations.
