Material for publication should be sent to the Editor, McWilson Warren, 3088 Briarcliff Road, Suite A11, Atlanta, Georgia 30329, USA. Such material should be accompanied by a letter from the corresponding author with signatures from all submitting authors indicating that they have participated in and concur with the submission and that the material has not been, and will not be, submitted elsewhere for publication as long as it is under consideration of the JOURNAL. Authors should disclose in writing any relationships or support which might be perceived as constituting a conflict of interest. Manuscripts and letters to the Editor should be typewritten, letter quality, in 10 point type, on one side of the paper and submitted in quadruplicate; if possible the manuscript should also be provided on disk. Paper should be 8 1/2 x 11 inches, pages should be numbered, and the manuscript should be neither stapled nor folded.
